These forms enable brands to collect essential information from employees before each shift. A common use is to confirm that proper attire is being worn. That said, this feature has broad application and can be used to address any aspect of your operations

SafetyForm Data
All data is recorded and stored in your PathSpot Dashboard (Forms & Checklists > Safety Forms), where you can access visualizations, apply filters, and download raw data for further analysis.
Safety Form Overview
All responses are aggregated by form, response category, time, and date, and presented through easy-to-interpret visuals
Safety Form Submission Data
Every employee response is available for download in a CSV file, which includes the following columns: Location, Department (if applicable), Form Title, Employee Name, Submission Status (Pass, Fail, Incomplete/Skipped), Date & Time of Submission, First Scan, and Last Scan.
This data can also be aggregated by clicking on 'Q&A Details' to view the percentage of passes per question, with filters available for date and location.
Employee Level Breakdown
For added granularity, you can drill down into each response after clicking on Q&A details to get specifics on how each employee answered
